Decision Guides for Life-Sustaining Treatments is an educational series designed by CCCC that explains complex topics associated with these difficult conversations using consumer-friendly language with evidence-based information.

The Guides were developed with the help and guidance of healthcare professionals, reflect the latest research on these interventions, and are intended to be used as a supplement to a conversation between a patient and provider.

High-quality prints of the Guides in English, Chinese and Spanish can be purchased in bulk from the CCCC Online Store, and are recommended for use by healthcare professionals who treat seriously-ill patients in hospitals, palliative care units, skilled nursing facilities, hospices and medical groups.
