The Coalition for Compassionate Care of California (CCCC) continues to grow our coalition-building efforts outside of California.
The Cognitive Decline Partnership Centre (CDPC), based at The University of Sydney, partnered with CCCC to adapt an advance care planning facilitator guide for use by staff in Australian senior centers, and independent and assisted living facilities. The original guide, Talking It Over, will be adapted and designed to raise awareness of the importance of early advance care planning for people diagnosed with dementia.
Talking it Over is a guide to facilitating advance care planning discussions among informal groups – such as congregations, community organizations, classes or senior groups – to explore personal views about end-of-life care. Though the discussion guide can be used by experienced facilitators and healthcare professionals, it is designed and written so that anyone can use it. CDPC is a research and knowledge exchange focused on improving health care for older adults experiencing cognitive and functional decline. The organization takes a coalition approach, bringing together clinicians, researchers, geriatricians, policy makers and consumers who have a wide range of expertise in working with people with cognitive and functional decline. Their recent report, Future Planning and Advance Care Planning: Why It Needs to Be Different for People With Dementia And Other Forms of Cognitive Decline, is available online.
