Pacific Business Group on HealthThe Coalition for Compassionate Care of California recently worked with the Pacific Business Group on Health (PBGH) to provide introductory palliative care training for California’s Health Home for Patients with Complex Needs (HHPCN). The Health Home program has several policy goals, including the goal to “integrate palliative care into primary care delivery,” per direction from the California Department of Health Care Services.

PBGH is a member organization of private employers and public agencies focused on increasing value and driving improvement in the American health care system.

The Medicaid Health Home State Plan Option, authorized under the Affordable Care Act, allows states to create Medicaid “health homes” to provide supplemental services that coordinate the full range of physical health, behavioral health, and community-based services and supports needed by beneficiaries with chronic and complex conditions.

In California, the Health Homes will serve eligible Medi-Cal beneficiaries with multiple chronic conditions who are frequent users of the system, and may benefit from enhanced care management and coordination. The program will provide six core services: Comprehensive care management; Care coordination; Health promotion; Comprehensive transitional care; Individual and family support; and Referral to community and social support services.

Under the California Medicaid Health Homes Program, PBGH provides technical assistance for practice transformation coaching for Community Based-Care Management Entities (CB-CME). CB-CMEs include clinics, physician offices, hospice agencies and others.

The palliative care education provided by CCCC included an orientation to palliative care definitions and services, differentiation of primary palliative care from specialty palliative care, and identification of resources.

    Will there be a public training program re this new initiative or is it for private health care agencies?
    If a public training is available please advise of dates.
