It is with great pleasure that we announce that four new local coalitions have been organized and selected as recipients of the Partnership HealthPlan of California (PHC)/Coalition for Compassionate Care of California (CCCC) Advance Care Planning Community Coalitions grant funding.

Advance care planning (ACP) is a critically important component in the delivery of person-centered and culturally appropriate care. The benefits of ACP are measurable and significant; yet, healthcare systems, staff, providers, and others face many challenges to engaging in or implementing ACP activities or programs. Higher levels of participation in ACP will benefit patients, families, and providers by reducing stress and clarifying goals. Additionally, ACP will promote care consistent with wishes and will reduce the costs associated with delivering care that is not in alignment with patient wishes.
Selected ACP grantees will receive 18 months of financial, leadership, educational, mentorship, and logistical support from PHC and CCCC to enhance and expand advance care planning in their communities. PHC will fund the coalitions with direct support for coalition development. PHC will also fund CCCC to lead the program management and technical assistance for the new ACP coalitions. Grantees will receive complementary membership in the Coalition for Compassionate Care of California, and all associated discounts and benefits, for the 18-month term of the grant.
