Hannah Whitehead is a clinical practice leader with Kaiser Permanente in Northern California focusing on improving serious illness care. She led initiatives to strengthen primary palliative care through ACP education, specialty tailored Serious Illness Communication training, and targeted identification of serious illness populations. With 20 years of experience caring for patients with serious illness, she is a board certified geriatric nurse practitioner and psychiatric mental health nurse practitioner. Her recent presentations include CCCC 2024, “Eliciting and honoring what matters most in the nursing home,” and AHPM 2026, “Primary Palliative Care Catalyst Program: A model for sustained culture change.”
