Jeffrey Yee is the division head of Supportive and Palliative Care with Mercy Medical Group - Dignity Health. He is board-certified in internal medicine specializing in Hospice and Palliative Medicine. He was the recipient of the 2014 Coalition for Compassionate Care Leadership Award and has presented at previous CCCC Annual Summits. He also contributed include on the original POLST Conversation Toolkit/Training, including the original “POLST Conversation” video, development of POLST FAQs, and POLST Clinical Case Studies.
