Jessica Zan is vice president of Clinical Implementations and Success at MyDirectives where she leads the design and implementation of advance care planning (ACP) workflows across health systems. Her work focuses on integrating ACP into routine clinical practice, improving access to patient preferences, and supporting team-based models of care. Previously, she developed and scaled ACP programs at INTEGRIS Health, embedding ACP into clinical workflows and training multidisciplinary teams. Ms. Zan is a frequent presenter on ACP at national and regional conferences, including Epic UGM, the SNP Alliance quarterly meeting, and the Florida Hospice and Palliative Care Summit.
