SESSION DESCRIPTION
UCLA Health created an outpatient hospice collaborative for information exchange to improve care. Since 2021, 12 hospices collaborated quarterly including data sharing and best practice discussions. A new EHR portal for real-time information permits hospices access to clinical information. Hospice enrollment displays in the EHR, is transmitted to the care team, and alerts ED and inpatient clinicians. Two-way communication and hospice updates are visible in the EHR. A registry of active hospice patients facilitates quality improvement. In this session, we will share how the project was accomplished and how it can be replicated.

- Describe quality measures for hospice care.
- Consider how a hospice data entry portal might be useful for their health system.
