POLST Champion Certificate Program (full program only)
Up to 11.0 CE/CMEs available for physicians/PAs, nurses, social workers, and chaplains. This is applicable only for those who register for and complete the full POLST Champion Certificate Program including all eight online Topics Modules, two Setting-Specific Modules, and in-person Skills Training.
Nursing: Provider approved by the California Board of Registered Nursing, Provider #CEP 15403, for
eleven (11.0) contact hours.
Social Work: Course meets the qualifications for
eleven (11.0) contact hours of continuing education credit for LMFTs, LCSWs, LPCCs and/or LEPs as required by the California Board of Behavioral Sciences. CCCC is approved by the California Association of Marriage and Family Therapists to sponsor continuing education for LCSWs and LMFTs, Provider #91987.
Chaplains: This program may be used for continuing education credit for chaplains certified with the Board of Chaplaincy Certification Inc.

General Information
Full attendance, in its entirety, is required at all educational sessions in order to receive professional continuing education credits (CMEs/CEs). Partial credit is not available. All attendees must register with their professional license numbers and use the unique Zoom link provided by CCCC in order for their attendance to be documented. Participants must also complete an online survey within 14 days following each training event in order to receive a CME/CE certificate or certificate of attendance. Continuing education certificates will be sent via email.
PLEASE NOTE: A 50-minute minimum of uninterrupted time is required to qualify for one hour of CME/CE credit. Continuing education excludes lunch and break periods.
Continuing Medical Education CLC and IB Standards (CLC IB Standards)
The California Legislature intended Assembly Bill (AB) 1195 and AB 241 to “encourage physicians and surgeons to meet the cultural and linguistic concerns” of California’s diverse patient population. Planners, faculty and speakers are central to adhering to state law as noted in Business and Professions (B&P) Code 2190.2, which codifies the aforementioned legislation. This standard focusing on patient populations allows for education to be designed to incorporate CLC components that best serve the provider’s patient population. The goal of these standards is to incorporate real-world scenarios into CME education to enhance physician competence in CLC, and for medical education to address how IB affects perceptions and treatment decisions that lead to disparities in health outcomes. IB in decision-making may contribute to health care disparities by shaping behavior and producing differences in medical treatment along lines of race, ethnicity, gender identity, sexual orientation, age, socioeconomic status, or other characteristics.
