Enhancing Medical Decision-Making Discussions for Healthcare Organizations
The Coalition for Compassionate Care of California (CCCC) works in partnership with healthcare organizations and communities to enhance advance care planning initiatives and programs through training and resource development.
We take a top-down as well as bottom-up approach, building from, and dovetailing with, whatever practices an organization already has in place. We start with what’s working, identify gaps and needs, and offer training and resources to enhance high-quality, sustainable programs.
Services
Our team provides expertise in the following areas:
- Strategic planning support
- Coalition building and leadership engagement
- Training curriculum with follow-up support and feedback for advance care planning and POLST facilitators and advocates
- Patient education materials development
We will help you evaluate existing strengths, weaknesses, and opportunities; build internal support and leadership engagement; and educate program advocates, staff, and key stakeholders about the benefits and processes of advance care planning and POLST. We will train staff regarding advance care planning best practices in order to establish systems and integrate them into ongoing care and evaluate program effectiveness.
