Jacob ‘Jake’ Quinton, MD, MSHS, FACP is a primary care internist, formerly NIH-funded health disparities researcher and the medical officer for the Patient Care Models Group (PCMG) at the Center for Medicare and Medicaid Innovation (CMMI) where he leads the equity strategy for PCMG and contributes to PCMG’s payment model design and implementation. Before CMMI, Dr. Quinton completed a health services research fellowship at UCLA (National Clinician Scholars Program) and previously trained at Yale New Haven Hospital in their Primary Care Internal Medicine program. His research as house staff into mortality risk prediction models used in the Veteran’s Health Administration won the Johns Hopkins GIM research award for population health research. 

Sibel Ozcelik, JD, MS
Medicare Advantage Value-Based Insurance Design Model Co-Lead
Center for Medicare & Medicaid Innovation (CMMI)

Sibel Ozcelik, ML, MS oversees the implementation, design and administration of the Value-Based Insurance Design (VBID) Model, which tests a broad array of complementary Medicare Advantage (MA) health plan innovations designed to reduce Medicare program expenditures and enhance the quality of care for Medicare beneficiaries. Prior to her role on the VBID Model, Ms. Ozcelik served as Special Assistant to the Chief Strategy Officer of CMMI and Senior Advisor to the HHS Secretary on Value-Based Care Transformation, supporting the strategy and development of service and payment delivery models, and supported development and operations of serious illness care research and models at Healthsperien, the Coalition to Transform Advanced Care (C-TAC) and C-TAC Innovations. Ms. Ozcelik earned an ML and MS from the University of Pennsylvania Carey Law School and University of Pennsylvania School of Social Policy and Practice, respectively.

Sarah Fogler, PhD

Sarah Fogler, PhD, Acting Group Director, Patient Care Models Group, Center for Medicare and Medicaid Innovation joined the Center for Medicare and Medicaid Innovation (CMMI) in February 2021 as the Deputy Director of the Patient Care Models Group (PCMG). Prior to joining CMMI, Dr. Fogler served as the Senior Director of Population Health and Community Benefit at Greater Baltimore Medical Center in Baltimore, Maryland. In this role, she led primary care transformation efforts, including implementation of a new behavioral Collaborative Care Program and Complex Care Clinic. She also previously held various leadership positions in the Center for Medicare, leading the policy development and implementation of the Medicare Shared Savings Program. In addition to Medicare fee-for-service payment policy, Dr. Fogler has also spent time working on Medicaid long-term care supports and services—namely supporting the Money-Follows-the-Person Grant program and the regulatory development of the Medicaid health homes. Dr. Fogler has an MA from Boston University, and a PhD from the University of Maryland, Baltimore County.
