The Coalition for Compassionate Care of California has partnered with CSU Shiley Haynes Institute for Palliative Care to develop self-paced, continuing education courses for healthcare professionals on the topics of advance care planning and POLST.

Effective Advance Care Planning: Skill Building For Everyone

This comprehensive course is designed to expand the workforce of skilled professionals trained to promote and facilitate advance care planning in all care settings. The courses detail critical components of advance care planning in the context of the new Medicare reimbursement.

Modules include:

  • Integrating Advance Care Planning into Your Medical Practice
  • Effective Advance Care Planning Fundamentals
  • Facilitating an Advance Care Planning Conversation
  • Billing for Advance Care Planning Conversations

The courses are ideal for:

  • Physicians, physician assistants, APRNs, nurses, social workers, geriatric care managers, medical technicians/assistants, discharge planners, chaplains, and billing specialists working in all care settings

Working With POLST For Professionals

This 2-hour online self-paced course is designed to help the healthcare professional understand the role and value of the POLST form, and how to employ it in documenting and honoring patient wishes.

This course provides specific details regarding POLST in California. The POLST form may help reduce some of that confusion when an Advance HealthCare Directive (AHCD) may not be accessible, wishes may not be clearly defined in the AHCD, or the AHCD is not a physician order.

Palliative Care For Care Managers

Care managers have an important role to play in helping health plans ensure that their members get the right care at the right time. Optimizing quality of care, maximizing member satisfaction and connecting to available community resources are all challenges that fall squarely on care managers.
