Community Engagement Toolkits

The Coalition for Compassionate Care of California (CCCC) has developed a number of toolkits that can be used to help engage a wide variety of communities in conversations about end of life and end-of-life care. Each toolkit provides step-by-step instructions on planning and publicizing a community engagement event around advance care planning. CCCC invites you to review these toolkits and use the one(s) that is best suited to your needs. 

END GAME Toolkit: Engage a wide and diverse range of communities – multi-ethnic and cultural, in rural, suburban and urban settings —in conversations about end-of-life and end-of-life care. 

Faith Leader Toolkits: Connect with faith leaders in their communities and provide the information and skills to help them support congregation members who are seriously ill, and their families and loved ones. 

The Road Ahead Toolkit: Provides an excellent way to reinvigorate an existing coalition or to jump-start a new one.
