Background: Research has shown that Chinese Americans continue to have the lowest rates of hospice use of any group. Only 5% of Chinese-speaking Chinese Americans say that they have “some” conversations about EOL wishes. In California, 60% of Mandarin or Cantonese speaking Chinese Americans say that they are very concerned about “finding health care providers who will understand and respect their cultural beliefs and values.” CACCC’s intentions are to build a community where Chinese Americans are able to face the end of life with dignity and respect.
Method: CACCC trained facilitators conduct group end-of-life discussions via CACCC’s Heart to Heart® Café. The Heart to Heart® Café was not only introduced to U.S. Chinese Communities and Healthcare providers, but also to the Healthcare providers in China and Taiwan. CACCC partnering with hospitals and hospices and trained volunteers to visit patients at those facilities.
CACCC offers Community Events on multiple topics: Advance Care Planning, AHCD Workshops, Health Care Options (utilizing CCCC Decision Aids), How to Start the Conversation (utilizing the Conversation Project Conversation Starter Kits), and is developing a new training course called “Mindful Self-Care for Caregivers” that will help family caregivers and their patients, volunteers, and healthcare professionals maintain their health, reinforce their confidence, and sustain their energy.
Results: CACCC was able to make a positive impact on the lives of 9,325 people that they had the privilege to engage directly and indirectly, addressing the health inequities in the Chinese population while preserving the dignity and independence of older adults, caregivers and the family unit. 87% of Heart to Heart® participants said they would discuss AHCD with their families or doctors.
Conclusions: Early engagement in ACP discussion, using culturally and linguistically appropriate materials is associated with a better understanding of EOL issues. The Heart to Heart® Cafe has had a dramatic effect on promoting ACP among Chinese Americans. Future research and funding should be directed toward interventions that address the specific needs of the Chinese American population identified in the Heart to Heart® Cafés.
