Lessons Learned From Native American and Rural Communities

COURSE DESCRIPTION

Native American and rural communities are overburdened with chronic conditions that disproportionately lead to terminal illnesses, yet these groups also have the lowest percentage of individuals utilizing supportive care services such as hospice and palliative care. YoloCares wanted to find out why, and how hospice and palliative care can be more effective in meeting the needs of these underserved communities. Among their surprising findings was that the best ambassadors for hospice were the spiritual care team, an outcome that may have applicability for how we introduce hospice and palliative care in other communities and cultures. As a devoted Gerontology Specialist and candidate for a master’s in public health, Brandy's journey began at bedside care, fueled by a passion to understand and address non-medical factors impacting disease progression in the life-long experiences of the under-advantaged elderly.  Through a blend of academia, research, and many years of experience serving elders through direct patient care, her focus extends to cross-cultural aging and dismantling systemic barriers that hinder optimal aging and transitioning for the most marginalized members of our society.

Currently serving as the Diversity, Equity, and Inclusion (DEI) Manager at YoloCares, her ongoing work involves culturally focused community-based Hospice and Palliative Care education, program development, cultivating organizational accountability, and fostering solutions for the myriad of barriers faced by marginalized communities. Brandy was the leader of the Life Transition Project, a community-based participatory research project identifying barriers to care for Native American and rural communities, and solutions to improve services.

  • Increase utilization of palliative care and hospice services in underserved communities.
  • Improve patient engagement strategies for patients from underserved populations who may be eligible for palliative care and hospice services.
  • Demonstrate cultural humility when designing and delivering services for Native American, rural, and other underserved populations.

CONTINUING EDUCATION

Full attendance at the live webinar (you must attend a minimum of 50 minutes) is a prerequisite for receiving professional continuing education credit. Attendees requesting CEU credit must each login into the webinar individually using the unique computer link from your instruction email for your attendance to be documented. If you do not follow these directions, there is no official documentation of your attendance, and you will not receive CEU credit for this webinar. Ask us about special arrangements offered to CCCC organization members.

You are also required to complete an online survey within 14 days following the webinar to receive a CEU certificate or certificate of attendance. A link to the evaluation will be provided in an email sent to you within 48 hours following the webinar. The email will also include links to the presentation recording and slides. Certificates will be sent via email. 

Nursing: Provider approved by the California Board of Registered Nursing, Provider #CEP 15403, for one (1) contact hour.

Social Work: Course meets the qualifications for one (1) hour of continuing education credit for LMFTs, LCSWs, LPCCs and/or LEPs as required by the California Board of Behavioral Sciences. CCCC is approved by the California Association of Marriage and Family Therapists to sponsor continuing education for LCSWs and LMFTs, Provider #91987. 

Chaplains: This program may be used for continuing education credit for chaplains certified with the Board of Chaplaincy Certification Inc.
