Why?
The state of California will soon launch a statewide POLST eRegistry; in preparation for this launch, each healthcare organization across the state should have a qualified POLST Champion expert to guide implementation.
Who?
The POLST Champion can be any clinician working primarily with patients with serious illness who would benefit from discussing, documenting, and communicating preferences for treatment in the POLST medical order set. Signing providers – MD/DO, NP, or PA – are especially appropriate for this role.
What?
The POLST Champion Certificate program includes (all these items must be completed to receive Certificate):
- 10 online training modules (8 topic modules and 2 setting-specific modules); all online training must be completed and passed prior to in-person training.
- In-person skills training (minimum of 20 participants per location needed to hold in-person training)
- Southern California, June 24, O’Connor Mortuary Chapel & Crematory, Laguna Hills (rescheduled from 4/29; registration cutoff is extended to June 15 - coursework MUST be completed by June 22)
- Northern California, October 14, DoubleTree by Hilton San Jose (registration cutoff is September 14)
- Examination
- Ongoing support/coaching and community
- Certificate continuation requirements

$669 CCCC Members / $799 Nonmembers
For groups of 3 or more registering at the same time:
$500 each CCCC Members / $639 each CCCC Nonmembers

CANCELLATION POLICY - FULL PROGRAM
As the majority of the Certificate Program is offered on-demand, there are NO REFUNDS available for cancellation. Continuing education credit is available as long as all requirements have been met.
