Transforming Pediatric Palliative Care Through Community and Academic-Medical Center Partnerships

SESSION DESCRIPTION

Despite strong evidence supporting pediatric palliative care (PPC), access to community-based services remains limited across California due to fragmented systems and workforce gaps. The Bay Area Pediatric Palliative Care Co-Lab unites UCSF, Stanford, George Mark Children’s House, By the Bay Health, Anchor Health, and CHoNC to build a seamless care continuum. This session highlights innovative pilots, early outcomes, and practical strategies for scaling coordinated, community-based PPC—offering attendees actionable tools to advance similar models statewide.

OBJECTIVES

  1. Analyze barriers to scaling community-based pediatric palliative care across health systems in Northern California.
  2. Design collaborative care models that expand access to home- and community-based pediatric palliative care services for seriously ill children and their families.
  3. Develop strategies to demonstrate the value of pediatric palliative care programs through outcomes data, patient and family experience measures, and economic analyses.
