The requirements to complete the POLST Champion Certificate Program include:

  • 10 online training modules
    • 8 Online Topics Modules
    • 2 Setting-Specific Modules
  • In-person skills training
  • Examination
  • Ongoing support/coaching and community
  • Certificate continuation requirements
 

Setting-Specific Modules

These modules focus on implementation of POLST in different healthcare settings. Each is one hour, including time for Q&A. Certificate participants must complete two (2) modules: one for the setting in which they are working and one “adjacent” setting.

The Setting-Specific Modules can also be attended as stand-alone courses worth 1 CE/CME each for those not participating in the Certificate program. The Setting-Specific Modules focus on implementation of POLST in different healthcare settings. Each one is one hour (30 minutes didactic content; 15-minute case study with examples of real-life situations presented by an expert; and 15 minutes of Q&A.
